What is Chronic Ankle Instability?

Chronic ankle instability is a long-term condition characterized by persistent looseness or instability of the ankle joint, usually on the outer (lateral) side of the ankle. It occurs when the ligaments that support the ankle become stretched or torn and fail to heal adequately after repeated sprains or a severe initial injury.

People with chronic ankle instability often experience repeated episodes of the ankle "rolling" or giving way, especially on uneven surfaces or during sports. If left untreated, the condition can increase the risk of additional sprains, cartilage damage, tendon injuries, and the early development of ankle arthritis.

What are the Causes of Chronic Ankle Instability?

Chronic ankle instability usually develops after ligament damage that does not heal completely. Common causes include:

  • Repeated ankle sprains. 
  • A severe ankle sprain causing significant ligament tears. 
  • Incomplete rehabilitation following an ankle injury. 
  • Returning to sports or physical activity too soon after a sprain. 
  • Weak ankle and lower leg muscles. 
  • Poor balance or reduced proprioception (joint position awareness). 
  • Participation in high-impact sports involving jumping, cutting, or rapid direction changes. 
  • Structural abnormalities of the foot or ankle. 
  • Untreated tendon injuries that affect ankle stability.

What are the Symptoms of Chronic Ankle Instability?

Symptoms often worsen with activity and may become more frequent over time. Common signs and symptoms include:

  • Repeated episodes of the ankle giving way. 
  • Persistent ankle pain or discomfort. 
  • Swelling that returns after activity. 
  • Tenderness along the outer side of the ankle. 
  • A feeling of looseness or instability. 
  • Difficulty walking on uneven surfaces. 
  • Reduced confidence during sports or exercise. 
  • Decreased range of motion. 
  • Weakness around the ankle. 
  • Recurrent ankle sprains with minimal trauma.

How is Chronic Ankle Instability Diagnosed?

Diagnosis begins with a thorough medical history and physical examination. Your healthcare provider will ask about previous ankle injuries, recurring symptoms, activity level, and situations in which the ankle feels unstable. During the examination, the ankle is evaluated for tenderness, swelling, ligament laxity, strength, balance, and range of motion.

X-rays are often obtained to rule out fractures, arthritis, or bone abnormalities. Magnetic resonance imaging (MRI) provides detailed images of the ligaments, tendons, cartilage, and surrounding soft tissues and helps identify ligament tears or associated injuries. In some cases, ultrasound may be used to evaluate ligament function during movement. Functional testing, including balance and stability assessments, may also be performed to determine the severity of instability and guide treatment planning.

What are the Treatment Options for Chronic Ankle Instability?

Treatment depends on the severity of the instability and the patient's activity level. Many individuals improve with conservative treatment, which may include rest, activity modification, ankle bracing, and anti-inflammatory medications to reduce pain and swelling.

Physical therapy is the cornerstone of treatment and focuses on strengthening the muscles around the ankle, improving flexibility, restoring balance, and enhancing proprioception. Customized rehabilitation exercises help improve joint stability and reduce the likelihood of future sprains.

Patients who continue to experience recurrent instability despite several months of non-surgical treatment may benefit from surgery. Surgical options typically involve repairing or reconstructing the damaged ligaments to restore ankle stability. In some cases, associated tendon injuries or cartilage damage may be treated during the same procedure.

Following surgery, patients usually undergo a structured rehabilitation program that includes temporary immobilization, gradual weight-bearing, strengthening exercises, and balance training. Most individuals can return to normal daily activities within a few months, while athletes may require additional rehabilitation before safely returning to sports. With appropriate treatment and rehabilitation, most patients achieve improved ankle stability, reduced pain, and a lower risk of future injuries.
